Xabi Alonso’s men are 16 points ahead of leaders Bayern Munich with six games remaining in the table and are still undefeated.
They will host Werder Bremen on Sunday at 4:30 p.m. Polish time. Live text updates will likely be available on the BBC Sport website and app, but the title could also be ready by then.
Bayern will host Cologne on Saturday, and one other defeat will mean that Leverkusen will win the league without having to play.
